To help you choose which papers is the best fit for you, please read the following:

~Patient Progress Report - This is only for patients who are having a re-examination done per the doctor's request. Please fill out the correct set of papers that match you.

~Updates - This is only for patients who have stopped care, are wishing to resume care and have not seen the doctor in less than three years. Please fill out the correcet set of papers that match you.

Both the Progress Report and Updates are included in each of the three sets.

~Set A - This is only for patients who see the doctor due to pain from the neck and shoulders and upper back. It contain 2 - 4 sheets that are necessary for you to fill out.

~Set B - This is only for patients who see the doctor due to pain in the lower back or lower mid-back. This set only contain 2 - 4 sheets that are necessary for you to fill out.

~Set C - This is for patients who see the doctor due to pain anywhere besides the low back, mid-back and neck and shoulders. This could be pain in the legs or arms. This set only contains 2 - 4 sheets that are necessary for you to fill out.

**** Please note that is it has been over three years since you have seen the doctor, you are considered a new patient once again, since the amount of time away from care has been so great. Please fill out the new patient forms if it has been three years or longer since your last visit to our office.****
